I (we) hereby authorize The Pacific Media Workers Guild, Local 39521 hereinafter called “THE GUILD”, to initiate debit entries to my (our) checking/savings account indicated below and the depository named below, hereinafter called “DEPOSITORY”, to debit same to such account.
This authority is to remain in full force and effect until “THE GUILD” and “DEPOSITORY” has received written notification from me (or either of us) of its termination in such time and in such manner as to afford “THE GUILD” and “DEPOSITORY” a reasonable opportunity to act on it.
How to pay your dues and initiation fee:
IMPORTANT CHANGE: Beginning June 1, 2016, all new members will be required to sign up for the auto payment option.
- Mail a check for your initiation fee with your printed and signed application and ACH auto payment form.
- Download, sign and email your application and ACH auto payment form to wmejia@mediaworkers.org, and send a check for the initiation fee to: Guild Freelancers, 433 Natoma St., 2nd Floor, San Francisco, CA 94103.
